Bourbon County Coffee Big Bad Baptist Coffee Bender They are the beers that have highlighted the coffee the most to my palate.
While it is a seasonal, I picked up four of the 2012 batch right before christmas and the 2013 batch just dropped this week. So you can usually find it year round. Fresh it's pretty good, but with a year on it this beer really shines.
You'll need to trade for it, or order it online, but Oakshire Overcast espresso stout is one of the most delicious of the coffee forward beers I love. Luckily for me a local brewery makes a stupendous one as well, Rushing Duck Planet Bean.

I would have agreed until I tried Peche Mortel and Speedway Stout, granted FBS is better than both of those brews. Then I tried Epic Big Bad Baptist... WOW!!... FBS is almost a memory. Damn, I better go buy a 4 pack! For me locally, there was also Elevator Bourbon Barrel Espresso Imperial Stout... Amazing stuff.
Alesmith Speedway Stout - really a coffee experience IMO. Wake&Bake FBS KBS - the BA cuts it a little, but it's still a tremendous brew.
BBC Coffeehouse is unreal on draft and awesome in a bottle. Wollavers/OtterCree Alta Gracia is really good. Harpoons new hundred barrel series Trifuno is awesome too.
This year's Hill Farmstead Earl is so much more coffee forward than the last iteration it's not even funny. Not just coffee but seriously high quality beans here.
Agreed on the beer. As far as its availability, it appears frequently on Letspour.com, and I'm pretty sure some other online sites have it as well (possibly MadeInOregon.com, for one). Most of the best/most obvious examples have already been named. For a beer that doesn't explicitly include coffee but I get a lot of dark roast coffee flavor from: Founder's Imperial Stout.
I'll second Real Ale Shade Tree Coffee Porter and Kona Pipeline. Both readily available (in DFW at least) and fairly inexpensive.

Flying Dog Kujo Imperial Coffee Stout might be my favorite coffee stout. It has such a nice/pleasant coffee taste and it doesn't even taste like it has 8.9% ABV. Such a smooth and creamy tasting beer.
I'm getting some in; let me know if you want to come to Chapel Hill and crack one! I can't guarantee how long they will or will not last though haha.
Hoping to pick up a couple bottles of the Hardywood Park Sidamo Coffee Stout this week. Will report back once I try one on for size. Right now, Cafe Royale from Williamsburg Alewerks is my leader.
The mother of all coffee foreward beers for me has been Peche Mortel. A dam near perfect coffee aroma. The coffee flavor was dam near perfect also. I am suprised noone has mentioned this beer. If you love coffee you will love this beer.
